Entry Requirement

Both the Application Document and the Method of Payment of Fees documents MUST be returned to the school. Thank you for considering MPHS (Midland Primary and High School) as an option for your child’s education.

Please note: MPHS is a Section 21 School which requires the payment of fees. School fees are a statutory debt. As of January 2014, the following documentation must be attached to the application forms before they can be fully processed:

• ● An unabridged birth certificate or proof of application.
• ● A clinic card.
• ● Annexure A (Method of payment).
• ● Both parents' ID documents.
• ● Proof of address (Only the original rates, electricity, and levy accounts, or your lease/deed of sale will suffice). Grade 1 applications require a copy of your Grade R account. We will make copies of the relevant pages.
• ● A copy of the most recent school report.
• ● Applications from non-South African citizens require a study permit.
